Are you concerned about forgetfulness, getting lost in familiar places, or difficulty following instructions? These experiences, along with slower thinking, can be normal with age. However, it’s natural to worry if you or a loved one experiences them. The challenge is knowing when to seek help.
The QMG Memory Clinic can help. We specialize in evaluating memory loss and other thinking changes in adults. Our team includes a geriatric medicine physician, neuropsychologist, nurse, and social worker. We offer a comprehensive evaluation including a physical exam, cognitive tests, and a family meeting to discuss the findings and create a plan.

The primary goals of the memory clinic are:
- To facilitate detection and treatment of dementia;
- To identify and treat potentially modifiable factors that may be contributing to thinking difficulties (e.g., medical conditions such as infections or metabolic changes, medication side effects, nutritional insufficiency, emotional distress, disrupted sleep, chronic pain, social isolation, etc.);
- To provide early diagnosis and treatment for progressive causes of dementia (e.g., Alzheimer’s disease and other neurodegenerative conditions); and
- To offer reassurance to individuals when changes are found to relate to healthy aging.
The Memory Clinic is designed to assess memory or thinking concerns in adults who do not yet have an established diagnosis. Patients with stable developmental disorders, acquired brain injuries, or a previously diagnosed neurodegenerative disorder should continue recommended care as advised by their primary care physician.

Preparing for your visit:
- Complete the Patient History Questionnaire prior to your visit. You can download the form here, or request a copy be sent to you in the mail.
- If possible, ask a family member or trusted individual who knows your current concerns to join you for this appointment in order to assist with providing background information.
- Bring insurance information to the visit. If you have any questions regarding insurance coverage for this visit, please contact your insurance company.
- Bring eyeglasses, contact lenses, and hearing aids to the appointment if you normally use them.
- Take your regularly prescribed medications on the day of your visit; do not use optional medications or substances that may cause drowsiness or alter thinking, such as allergy, pain, or anti-anxiety medication, alcohol, or cannabis on the day of your visit.
