Are you con­cerned about for­get­ful­ness, get­ting lost in famil­iar places, or dif­fi­cul­ty fol­low­ing instruc­tions? These expe­ri­ences, along with slow­er think­ing, can be nor­mal with age. How­ev­er, it’s nat­ur­al to wor­ry if you or a loved one expe­ri­ences them. The chal­lenge is know­ing when to seek help.

The QMG Mem­o­ry Clin­ic can help. We spe­cial­ize in eval­u­at­ing mem­o­ry loss and oth­er think­ing changes in adults. Our team includes a geri­atric med­i­cine physi­cian, neu­ropsy­chol­o­gist, nurse, and social work­er. We offer a com­pre­hen­sive eval­u­a­tion includ­ing a phys­i­cal exam, cog­ni­tive tests, and a fam­i­ly meet­ing to dis­cuss the find­ings and cre­ate a plan.

The pri­ma­ry goals of the mem­o­ry clin­ic are:

  • To facil­i­tate detec­tion and treat­ment of dementia;
  • To iden­ti­fy and treat poten­tial­ly mod­i­fi­able fac­tors that may be con­tribut­ing to think­ing dif­fi­cul­ties (e.g., med­ical con­di­tions such as infec­tions or meta­bol­ic changes, med­ica­tion side effects, nutri­tion­al insuf­fi­cien­cy, emo­tion­al dis­tress, dis­rupt­ed sleep, chron­ic pain, social iso­la­tion, etc.);
  • To pro­vide ear­ly diag­no­sis and treat­ment for pro­gres­sive caus­es of demen­tia (e.g., Alzheimer’s dis­ease and oth­er neu­rode­gen­er­a­tive con­di­tions); and
  • To offer reas­sur­ance to indi­vid­u­als when changes are found to relate to healthy aging.

The Mem­o­ry Clin­ic is designed to assess mem­o­ry or think­ing con­cerns in adults who do not yet have an estab­lished diag­no­sis. Patients with sta­ble devel­op­men­tal dis­or­ders, acquired brain injuries, or a pre­vi­ous­ly diag­nosed neu­rode­gen­er­a­tive dis­or­der should con­tin­ue rec­om­mend­ed care as advised by their pri­ma­ry care physician.

Prepar­ing for your visit:

  • Com­plete the Patient His­to­ry Ques­tion­naire pri­or to your vis­it. You can down­load the form here, or request a copy be sent to you in the mail.
  • If pos­si­ble, ask a fam­i­ly mem­ber or trust­ed indi­vid­ual who knows your cur­rent con­cerns to join you for this appoint­ment in order to assist with pro­vid­ing back­ground information.
  • Bring insur­ance infor­ma­tion to the vis­it. If you have any ques­tions regard­ing insur­ance cov­er­age for this vis­it, please con­tact your insur­ance company.
  • Bring eye­glass­es, con­tact lens­es, and hear­ing aids to the appoint­ment if you nor­mal­ly use them.
  • Take your reg­u­lar­ly pre­scribed med­ica­tions on the day of your vis­it; do not use option­al med­ica­tions or sub­stances that may cause drowsi­ness or alter think­ing, such as aller­gy, pain, or anti-anx­i­ety med­ica­tion, alco­hol, or cannabis on the day of your visit.
