Foundation Funding Report

Funds must be expend­ed with­in one year of check date. Orga­ni­za­tions will not be eli­gi­ble to receive future fund­ing if the Fund­ing Report is not sub­mit­ted to the Quin­cy Med­ical Group Foun­da­tion. Incom­plete reports will pro­hib­it con­sid­er­a­tion of fur­ther donations.
